Brooke Tabbert, 30, of Oak Harbor, died Saturday after a two-vehicle crash at the intersection of Five Point and Lime City roads in Perrysburg Township. The Ohio State Highway Patrol said the crash happened about 4:49 p.m. April 4. Tabbert was driving a 2018 Ford east on Five Point when she failed to yield at the stop sign at Lime City Road. A southbound 2016 Ford SUV driven by 29-year-old Clemente Vallestero of Perrysburg struck her vehicle. Both vehicles ran off the southeast corner of the intersection and slammed into a utility pole.
First responders rushed Tabbert to Mercy Health St. Vincent Medical Center, where she was pronounced dead. Her infant, who was in the vehicle, was taken to the same hospital with minor injuries. Vallestero suffered minor injuries at the scene and was not transported to a hospital.


The Ohio State Highway Patrol’s Bowling Green post is handling the investigation. Perrysburg Township Police, the township fire department and local tow companies assisted at the scene.
